Dr. Dalheim is not a participating provider in insurance panels; however, she will provide paperwork for the patient to submit for reimbursement from plans with out-of-network benefits. Patients with Medicare may inquire with Dr. Dalheim as to whether she is taking new patients. Medicare/Medicaid is not accepted.

Payment is expected at time of service and credit cards are accepted.
